idk wat to say to this guy, but 4real for your kids i would quit, cuz u obviously know it destroys lives and i kno u kno its bad and i understand ur addicted, i only live with my dad and if anything would happen to him i wouldnt kno how to feel, or deal with it so...just try to quit man its the right thing to do.
but like all other things in life ITS GONNA BE HARD AND A TIME PROCESS AND ITLL BE WORTH EVERY MINUTE AT THE END...trust me

things that take the most focus and time and pain end up to be worth it the most.
its not easy, but its worth it.

How you would do it? Hell I don't know. I can't quit smoking, drinking, or jerking off. Hmm, I'm reading the second book of The Dark Tower, let me see how he saves this guy from HEROIN and I'll let you know. Just kidding man, best of luck to you, enjoy life.
