these days i'm usually smoking alone or with one other person. but if three or more are involved, i'd say to the left - in most cases. sometimes though i'll just pass it to who's ready or who i want to pass it to (my gf if it's my weed, my friend that just packed one...etc) i'll pass it to someone that pitched in weed on my right, over someone that is mooching on my left.
we went by peace pipe to the left when we were younger..

my ex gf would always pack a bowl with her weed, give it to the person on her right to spark, and then take the second hit herself. that way it always went to the left.

 

also i smoke a lot around other people that don't smoke, so i'd pass it to a smoker on my immediate right before i pass it to a smoker on my left but separated by a nonsmoker.

basically who ever is closest/more deserving of being passed to if it's a large crowd.
