So I got an anal plug that seems the right size and has a t bar base that will be great for wearing out and about.

However the seam that runs around the plug from top to bottom has a couple of sharp points. One on the tip and one half way down.

I want to get rid of these so there is no damage caused internally but I think sanding down the parts will end up ruining the otherwise smooth surface of the toy.

Any suggestions on how to smooth out these sharp parts? It's a semi-hard silicone, not rock hard and not really soft jelly.

Try a very sharp non-serrated knife held 90° to that of which you want to remove. The idea is not to cut but to scrape instead. This takes some downward pressure while scraping and may take several or more passes.
