Clinical applications of LSD and Shrooms!

    Hey guys,
    Im running a research project as part of my dissertation at University and need to collect data on individuals who have taken LSD or mushrooms in the past 12 months. It you fit this please help!
    I have attached below a link to our survey, it will only take 5mins!
    All data we collect is completely ANONYMOUS, so please be honest!
