Outside of STD worries there is a lot of bacteria inside the rectal cavity. With or without a condom this bacteria can be passed from one to another. The bacteria can spread infection but probably only requiring antibodies' to correct it. it doesn't matter if everyone being penetrated has performed an enema some bacteria remains. The real issue is membrane tears inside the rectal opening happening because of penetration without enough lubrication.
