considering you see yourself every waking hour it's hard to see small processes like getting fat. I used to be fairly overweight and lost a considerable amount of weight. I don't really remember how I looked when I used to have all that extra weight, but it feels as if I'm the same. When others see me their jaw drops, and also if it wasn't for people telling me I was getting fatter I never would have noticed it and gone on with the initiative. So to answer your question some people don't see themselves getting fatter because it's hard to notice when you're you; but it takes a person that wants to help you to remind you that the extra fat is taxing your beautiful body.
Really big people tend to have very strong legs. Compare it to a healthy weight person walking around everywhere with a hundred or two pound backpack on
I've always kind of felt bad for really large people regardless of how they got that way. It may or may not be their fault and they may or may not be taking steps to lose weight but I would guess that it bothers many of them every day. I'm not talking about people who could stand to lose 20 or 30 lbs. I'm talking about people like the lady I saw today who would swim in her skin at a normal weight if she lost 160 lbs.

People over eat, and they don't have balanced diets. They don't have nearly enough activity in their lives. I say "activity" instead of exercise because the neurologist in my office recommends activity (i.e. walking, taking the stairs vs escalator and elevator) more than he does rigorous exercise. He walks 4.2 miles a day (2.1 to work, and 2.1 to his home). My daily commute to work requires that I walk 1.2 miles. (0.6 there, and 0.6 back). Then whatever walking around I do throughout the day.
There is more to obesity than simply eating too much and not getting enough exercise. Obesity is mostly hormonal. It doesn't have as much to do with too much food and too little exercise, as much as what a person is eating, combined with some environmental factors as well. You never see obese animals in the wild. My point is that obesity is not a natural state. diet and activity can balance your hormones its important to know that hormones can be controlled naturally and they do not act alone. diet and activity level directly effect hormones. For example, when a woman has either too much estrogen or too little.. a doctor will recommend a certain diet plan for a month or two. And an increase in physical exertion can cause higher testosterone in women (same for men), and lower estrogen levels. Weight gain and obesity is associated with high estrogen levels. Also, some body fat reduces testosterone levels but the opposite effect is not reached by simply increasing the hormone. Men with sedentary lifestyles tend to have higher estrogen levels and lower in testosterone.
Well of course, our bodies are complicated. Diet and activity effect far more than our digestive system and outward appearance. They contribute to a healthy mind and happy heart. Which are also needed for healthy weight managing.
There is a certain acceptance of obesity in our culture also; for example, skinny shaming ("That girl needs to eat a hamburger," etc) and stupid slogans like "big is beautiful" and "real women have curves!" Sometimes people know they're fat and embrace it rather than admit it is due to an unhealthy lifestyle. Also, most people don't really have any working knowledge of basic nutrition, and a lot of people live really sedentary lifestyles and don't really know how to change.
