How do I go about asking...

for an appointment to get a card?
Until now, I always assumed that if I went in for a checkup and said 'Doctor, I've had this pain in my lower back', he'd recommend I get smoke some weed, lol
Is there a special process? A form to fill out?
It'd also be awesome and make my day if I had an idea of the prices, thank you guys!

if your regular doctor isnt cool with it, then you can seek a specialist, because you informed your regular physician that you want the card.

Most of the clubs require pre registration. Meaning after you've gotten your card call the local dispensary and tell them you want to join. Usually they'll verify your card with the doctor's office and that's that.
