It's speculation, but it seems that the disease in Trump worsened rapidly after what appears to have been only a few days of visible symptoms (his rough voice, lethargy, etc). He was given steroids which are normally used when the disease is in a later stage so as to not hamper the body's natural immune response. It's possible that his steroid use was justified because Trump was also on experimental medications that boost the immune system and an antivial medicine. Those may reduce the chance of possible negative effects of the steroid on his immune system and allow the steroid to reduce inflammation in his lung area. Dr. Conley has evaded questions about the condition of Trump's lungs, which gives the impression that some pathology may have been present. Conley was initially evasive to questions about steroid use but later acknowledged Trump was on steroids. Conley and others have evaded when Trump's last negative virus test result occurred. That is crucial to knowing when the virus was replicating enough for a test to register positive which indicates when the virus was present in earnest. It matters greatly to those who were around Trump at the time near the last negative test who could have been infected by him. It's difficult for those on the outside to assess Trump's condition when his doctors have been quiet about critical data.
