Breakthrough Drug for Covid-19 May Be Risky for Mild Cases

"Patients given the steroid who were not receiving respiratory support, however, actually died at a slightly higher rate than similar patients who were not given the drug, although the difference was not statistically significant.

That the drug might have disparate effects at different stages of the disease makes sense, given the biology of the illness, according to Dr. Martin J. Landray, the study’s senior author and a professor of medicine and epidemiology at Oxford University.

In the most severely ill patients, the immune system flies into a harmful overdrive mode, triggering a so-called cytokine storm that damages the body, including the lungs.

“It’s almost like a disease with two phases,” Dr. Landray said in an interview. “A phase where the virus dominates, and the immunological phase, where the damage the immune system causes is dominant.”

Doctors have worried about using steroids such as dexamethasone to fight Covid-19, because steroids are anti-inflammatory drugs that dampen the body’s protective immune response. In mildly ill patients, that may do more harm than good, Dr. Landray said.

“In the early phase of the illness, the immune system is your friend,” Dr. Landray said. “It’s fighting the virus, and dampening it is not a good idea.”

“In the later phase, the immune system is no longer your friend — it’s responsible for the lungs failing, and dampening it down with steroids helps the situation and improves the chance of survival.”"

 

It's speculation, but it seems that the disease in Trump worsened rapidly after what appears to have been only a few days of visible symptoms (his rough voice, lethargy, etc). He was given steroids which are normally used when the disease is in a later stage so as to not hamper the body's natural immune response.

It's possible that his steroid use was justified because Trump was also on experimental medications that boost the immune system and an antivial medicine. Those may reduce the chance of possible negative effects of the steroid on his immune system and allow the steroid to reduce inflammation in his lung area.

Dr. Conley has evaded questions about the condition of Trump's lungs, which gives the impression that some pathology may have been present. Conley was initially evasive to questions about steroid use but later acknowledged Trump was on steroids.

Conley and others have evaded when Trump's last negative virus test result occurred. That is crucial to knowing when the virus was replicating enough for a test to register positive which indicates when the virus was present in earnest. It matters greatly to those who were around Trump at the time near the last negative test who could have been infected by him.

It's difficult for those on the outside to assess Trump's condition when his doctors have been quiet about critical data.

At least 6,000 people attended meetings, rallies and other gatherings with them within a week of the Supreme Court nomination ceremony Sept. 26 in the White House Rose Garden, pegged as a potential “superspreader” event."

After losing in the 9th Circuit, Trump asks Supreme Court to allow it to halt census count, apparently because Trump wants to reduce the count of the number of minorities which includes legal and undocumented immigrants.

Trump lost in the Supreme Court when he wanted to include a question on the census that asks the U.S. legal status of a person.

Christie was seen standing close to people at the event and offering handshakes.

Manhattan D.A. Can Obtain Trump’s Tax Returns, Judges Rule

The Manhattan district attorney can enforce a subpoena seeking President Trump’s personal and corporate tax returns, a federal appeals panel ruled on Wednesday, dealing yet another blow to the president’s yearlong battle to keep his financial records out of the hands of state prosecutors.

The unanimous ruling by a three-judge panel in New York rejected the president’s arguments that the subpoena should be blocked because it was too broad and amounted to political harassment from the Manhattan district attorney, Cyrus R. Vance Jr., a Democrat.

“Grand juries must necessarily paint with a broad brush,” the judges wrote.

They concluded that the president did not show that Mr. Vance had been driven by politics. “None of the president’s allegations, taken together or separately, are sufficient to raise a plausible inference that the subpoena was issued ‘out of malice or an intent to harass,’” they wrote.
