so I print and fill that out. then I get a doc to sigh it? I know of a doc that only dose pot. I just have to find his number and call

 

the first link is the application .....you must print it out and mail it.....there is no online registration because you need legal signatures on the document

ok so I got my medical records but its in one folder and there is 151 pages I hope to god I don't have to print out all that shit just the ones that talk about my Migraines and abnormal eeg and scoliosis all he has to do is look at my back and that says it all right there.

you photocopy the records you need and send copies of the actual one supplied by the doctors office

and ask if you have done it correctly...explain you have difficulty with written words and they should have staff to assist you

ok thanks actually my mom is the one doing all of this she printed off all of the ones I need. and we found a dispensary that has prices and where to go

 

update bad news there clinic is only open on Saturdays and my mom works every Saturday I hope she can some how get that day off. its 180$ they do all the paper work we have to mail it in ourselfs bring medical records he looks at them and sighs off on it 2-4 weeks it takes too get my card

 

I mad they appointment for that Saturday my sis has off so she said she could take me only probe my nieces might be with so I have to go in alone to the doc and they wait in the waiting room
