Hi.. I'm decidedly technology ignorant, so please forgive me if I'm asking something that has been answered elsewhere (it would be greatly appreciated if that's the case to give me a link).... Simply, I'm a long term chronic pain patient (I know, me and 297 trillion others... the statistics are incomprehensible to me) and would be SO grateful if anyone knows of a pain management clinic/doctor/shaman anywhere in the nyc area where they are willing to treat and prescribe opiates for a stable, long term ( about 7 years), oxycodone patient that doesn't use any illegal drugs, and kinda sorta really doesn't want to seek out my local H dealer to start on that road. My internal medicine physician wants me to find someone else to treat me (thankfully, he won't just cut me off), and up my dosage (by about 10-5%, having been on my current dosage for about 2 years). Any all referrals appreciated more than I have the ability to express in words. Thank you, D
Your doctor probably has a better idea who to talk to then this forum if you have legit uses for pain meds. If you want it for recreational reasons it's just a matter of talking to doctors until they tell you what want to hear. Don't attempt to treat anything with heroin.
You might be able to reduce your pain significantly with acupuncture, or possibly with an acupressure self-massage, in which case there would be no need to increase your pain meds.
Unfocused, my doc, though incredibly sweet, is totally overwhelmed professionally. I, of course, asked him for guidance on this, but, he basically said words to the effect of "The DEA has been intruding on my practice, I have no idea who will take this over for me (my management), it's up to you to find someone. Newbie one, I have been through, in no particular order, physical therapy, extensive acupuncture, a complete endocrinological workup and all the medications associated with it, NSAIDS, etc. The only thing I won't let them do (which has been recommended) is to fuse my metatarsal and phalanges (obv, my problem are my feet, both totally gone). They suggested this fusion a couple years ago, but, agreed there was no assurance the pain would abate (I've had multiple surgeries on both feet, and, they are totally arthritic, devoid of cartilage. The second set of surgeries was done by a podiatric specialist, who told me, during follow up care... "I can't believe you can even walk on those things". I wish it weren't the case, but, oxy is the only thing that allows me to be ambulatory. Thank you both for your responses, I'm just at a loss to find a doctor anywhere that can deal with this (so far, I've been to 3 board certified pain management specialists, who basically told me the same story (too much heat, find someone else).
Pain meds, whether legal or illegal, are a path to increasing tolerance and dependence. You may also have a hard time finding anyone who is willing to up your dosage. I can't really help with any pain med recommendations, I can just go over some alternatives, though I appreciate that in principle increasing meds may be the only effective route. Who gave you acupuncture, and how many treatments did you get? Sometimes they let MD's do acupuncture with minimal training, even though they don't know what they're doing. I would try to only see someone with 4 year degree in acupuncture from a good school. Acupuncture has a reputation for being very good with arthritis, but often the progress is slow. Unless you spent a couple of years getting it with no positive results, it might be worth trying again. Chinese herbal medicine can also be good for arthritis. Some dietary changes might help as well. Do you have hot or cold feelings that come with the arthritis? Heavy limbs? Pain that comes and goes?
Newbie, Thank you so much for even taking the time to respond. I appreciate your suggestions, and I'll respond as best I can. First, my condition, simply: the cartilage between my metatarsal and first phalange (bilaterally) is gone. The other toes are degraded, but not to that degree. So, it's bone scraping bone. The pain is constant, it only changes in degrees (from a "2" or so when waking, as, I've not been on my feet, to an 8-9 after running around after my 5 year old). I have been interested in non traditional medicine and modalities for over 4 decades. I'm familiar with and have tried herbal treatments, my acupuncturist was Chinese, had 5 decades of experience, and said after a couple of months that he knew nothing that would help (there wasn't even the slightest improvement). He was, also, extraordinarily compassionate and skilled (he essentially changed my wife's periodic cycle, which allowed her to become pregnant... not bad!). I am familiar with acid based diets, and how they exacerbate inflammation; I take curcumin and other herbs regularly, which perhaps help a tiny bit, it's hard to judge improvement, as if so, it's incremental. I have no other symptoms of arthritis other than the direct and maddening pain. No flashes, no intermittent pains elsewhere (I did have a hip replacement- it runs in the family, and that came out perfect). My condition (as theorized by my surgeons) is the result of over 3 decades of loving and playing tennis. If I had the funds, I'd very very strongly consider Ibogaine treatment (if you're not familiar with it, check it out) at one of the few Dr. attended clinics; it sounds pretty miraculous. At any rate, I just want to say thank you for caring enough to write. Sometimes, I do fear I'll go mad... but, there are so many that suffer so much worse, it'd be self indulgent.
You're welcome. I hope that I can help in some way. You might want to consider using a cane, a walker, or a wheelchair. While I'm sure you wouldn't want to give up your mobility, being an opioid addict (or experiencing increased pain from walking or running) may be a worse fate. In principle, spending a few months with minimal time on your feet could allow tissue to grow back. I'd recommend seeing a chinese herbalist.
