I’ve heard of stories where a woman would forget to take out the sponge after sex; does that also mean it’s possible to forget to put it in when having sex regularly? Does it have to be changed after each sexual encounter?

I never forgot the sponge was in there. They say you can put it in up to 24 hours before having sex and you need to leave it in for 6 hours after sex, so you can have it in for 30 hours per the instructions (but not more than that) I sometimes would put it in early evening and and leave it in until the next morning, about 12-14 hours total. I never left it in for more than that. You can leave it in and have sex several times as long as you stay within the 24 hour time frame. It doesn't absorb and hold all the semen so there is some leakage.
