I don't think that fetishes are necessary to experience pleasure, no matter what the level. I think, that if anything, fetishes come up out of a lack of pleasure. You were deprived of this or that, and then you saw something that you connected with as a source of salvation. That salvation becomes your new source of pleasure, your new fantasy. There's no reason why you can't experience bliss, or reach new heights of enjoyment, without a specific focus of pleasure.
